Grievance/Incident Submission Form
Dispute Resolution Procedure
AVAHA has created this page as a resource to help parents/coaches/players or other members of the organization resolve grievances. A “grievance” is defined as any conflict; dispute or disagreement between Participants, including any parents or guardians of a player and that player’s coach or a league official or members of different AVAHA teams that allege a violation of AVAHA Policies & Procedures and/or USA Hockey rules and policies. All Participants should attempt to resolve grievances expeditiously and fairly at the lowest possible level within the Dispute Resolution Procedure. These procedures are not, however, intended to resolve minor issues related to a player’s participation on a team (such as playing time, positioning, or minor discipline) or private disputes between Participants.
Grievances should allege a specific violation of the AVAHA, UAHA, or USA Hockey rules, policies, or procedures. Allegations of Safe Sport Violations shall follow USA Hockey’s Safe Sport Program reporting procedures and shall not utilize the Dispute Resolution Procedures.
If you suspect a SafeSport violation has occurred, stop now, consult the Safesport guidance and reporting options at:
If you suspect a criminal offense, contact law enforcement immediately and report to USA Hockey using the Safesport report submission tool above.
If this is not a SafeSport violation please continue below to steps 1-3
Step one:
The 24-Hour Rule: Parents/guardians are not allowed to confront a coach, team, and/or league official to discuss any “negative” game and/or practice situation with the coaching and management staff until at least 24 hours has passed from the completion of the game and/or practice. A confrontation shall consist of any conversation, which is elevated from a normal speaking tone and demeanor to one which involves yelling, profanity, and/or derogatory comments toward said coach, team, and/or league official. Violation of the 24 Hour Rule Policy is considered a violation of the USA Hockey Zero Tolerance Policy.
Step two:
Grievances about a team, its players or its coaches should first be presented verbally to the Team Manager and addressed between the parties involved, in a spirit of cooperation. Team Managers should work with the members, coaches and/or other team officials to resolve the grievance internally, in an expeditious and fair manner. Coaches are encouraged to meet with members to attempt to resolve grievances in an amicable and informal manner. If a grievance arises between other AVAHA members of different teams or AVAHA events the issue should be verbally expressed to a board member. If the grievance is not resolved in a meaningful way, the subject should then be addressed by Step Three.
Step Three:
If the grievance/incident has not been resolved and you have completed step one and step two, please fill out this form to notify the AVAHA Board of Directors.
